Calgary doctor suspended over illegal Botox injections

A Calgary doctor has been sanctioned after he admitted to administering Botox injections to patients without a permit.

Dr. Michael Prevost was suspended by the College of Physicians and Surgeons of Alberta (CPSA) for nine months after he provided patients with the treatment for migraines.

According to the CPSA, Prevost was only permitted to practice in esthetic medicine and hair transplantation.

The tribunal decision, dated Feb. 4, said Prevost administered the treatment 114 times over a three-year period.

The CPSA said he also submitted 42 billings for Botox injections between September 2021 and January 2024.

Prevost sanctioned in 2022

This latest penalty is the second time Prevost had been sanctioned for practicing medicine outside what his permit allowed.

The CPSA tribunal, in a 2022 decision, said he provided Botox injections on approximately 79 occasions to patients suffering from migraines.

“Dr. Prevost continued to engage in this conduct even after signing an admission in the earlier CPSA hearing on November 2021, in which he admitted that he had provided Botox injections to patients for the treatment of migraine headaches in contravention of the conditions of his practice permit restricting him to only esthetic medicine and hair transplantation, and that his conduct amounted to unprofessional conduct,” the CPSA said in the most recent decision.

“There is serious harm done to the profession when a member disregards conditions that have been imposed on his practice permit, made worse when a member continues to engage in conduct that he has admitted is unprofessional conduct.”

In addition to the suspension, Prevost was ordered to complete and unconditionally pass a course on ethics and professionalism and pay the costs of the hearing to a maximum of $7,500.

His suspension began in December 2025.
