On Easter Monday, Toronto Mayor Rob Ford had security guards bring an unknown woman to his empty downtown office.

Security guards arrived at the mayor's office with the unidentified woman at approximately 7:55 p.m. on Monday, April 21, according to a report filed by City Hall security and obtained by CTV Toronto.

"The Mayor appeared to be intoxicated as he was slurring his words, red in the face, and sweating profusely," the report said.

Security officials have not said what happened with the woman, or when she left, but the mayor was picked up two hours later by his brother, Coun. Doug Ford.

The names of the woman and the city employee who filed the report have been redacted.

CTV Toronto and the Toronto Star also obtained copies of a security camera video from the same night by filing a freedom of information request.

The video shows an incident in the parking lot after the woman left.

Shortly before 10 p.m., the mayor and his brother, Coun. Doug Ford, can be seen entering the parking lot where Rob Ford's Cadillac SUV was parked.

Security guard Mike Toolaram could be seen helping the mayor into the passenger's seat of the Escalade while Doug Ford waited behind the wheel.

"The Mayor can be seen stumbling and even falling on the ground before being seated in the vehicle," the report said of the video.

The security guard then shut the door, and the SUV drove out of frame.

The events seen in the video footage were also were detailed in the security guard's incident report.

The incident occurred about a month after the mayor was criticized for showing up at city hall on St. Patrick's Day weekend, when he allegedly threatened a security guard who had reported seeing the mayor apparently intoxicated on the same holiday in 2012.

Less than two weeks after the Easter Monday events, the mayor announced he was taking a leave of absence to seek professional help for alcohol abuse problems.

"I have a problem with alcohol and the choices I have made while under the influence. I have struggled with this for some time," the mayor said in a statement released on May 28.

Ford checked into a rehab facility in Bala, Ont., where he has been getting treatment. Doug Ford told CTV Toronto that the mayor will leave rehab on June 30, and will return to work the next day.
